Bridge Medical Centre - Information for new patients wishing to join the doctors surgery

Registration

You may register as a patient by calling at our front desk where you will be asked to complete a new patient questionnaire.

You will need to be resident within our practice boundaries and you will be asked for proof of residency and photo id. You will be offered the opportunity to receive a Health Check consultation with one of our Health Care Assistants.

As part of the normal registration process you will be registered as a patient of the practice. You do, however, have the opportunity to state a preference to receive services from a specific practitioner. Where this request is made we endeavour to comply, however we do reserve the right to refuse your request if we feel we have reasonable grounds to do so.

We register prospective patients if their postal address falls within one of the following areas:-

NORTHGATE, THREE BRIDGES, POUND HILL, WORTH, MAIDENBOWER, FURNACE GREEN, TILGATE AND COPTHORNE

Pre-Registration with the practice

PreRegistration If you wish to pre-register to save time, click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.

We will be able to complete your registration process when you visit the surgery for the first time. You will be asked to sign the form to confirm that the details are correct and to show proof of residency and id. You will not be registered with the practice until you have completed the process at the surgery.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Extended Hours

As from 1st January 2011 we have been providing additional appointments outside of regular surgery hours. These are be pre-bookable appointments only, and the surgery is not open for any problems or administration requests at these times, INCLUDING COLLECTING PRESCRIPTIONS.

The telephones will remain diverted to Harmoni out-of -hours. We are providing a doctors surgery every other Saturday morning, along with one late surgery per week.
